Abstract
Ankylosing spondylitis is a multisystem disease. It's extraskeletal manifestations
contain fibrobullous lesions in upper lobes of lung which are confused with pulmonary
tuberculosis.
We report a case of 59 year old man with ankylosing spondylitis who has fibrobullous
lung lesions which was confused with pulmonary tuberculosis.
On radiological examinations, he had bamboo spine, kyphosis and severely narrowed
sacroiliac joint space.
HLA B-27 was positive. He had fibrobullous lung lesions in both upper lobes which
had not progressed for 6 years without antituberculosis medications. And then he took
antituberculous medications for 2 years, but symptomatic and radiological improvement
were not developed. Multiple sputum AFB stains were negative.
So, we conclude that he has fibrobullous pulmonary involvements of ankylosing
spondylitis.
